Data for retirement plan design

We would be happy to look into some retirement plan alternatives for your company, but first:

  • tell us what you'd like the plans to accomplish, and
  • pull together the data we'll need to crunch the numbers.

Information on your existing plans - summary plan description, plan document or adoption agreement.

Employees included - all employees and owners who have met the age and service requirements to enter the 401(k), profit sharing, pension or money purchase plan - whether they are included in the plan or not. If your plan requires employment on the last day of the year to receive an allocation, you may exclude employees who terminated during the year with less than 500 hours of service.

Individual data (provide all relevant data items; we always need pay and birth date)

  • Name or other ID
  • Group code - to identify owners or other groups to be eligible for different contribution rates
  • Union code - use "U" for union members
  • Key employees - use "K" for keys, "N" for others
  • Highly compensated employees - use "H" for HCE's, "N" for others
  • Sex (M or F - for defined benefit pension plans only)
  • Birth date (MMDDYY)
  • Hire date (MMDDYY)
  • 401(k) eligibility date (optional - MMDDYY)
  • Profit sharing/money purchase eligibility date (optional - MMDDYY)
  • Termination date (if terminated - MMDDYY)
  • Hours worked for the year (show hours or a code to indicate <500, 500-999 or 1000+)
  • Annual pay used to calculate contributions (e.g. base pay, excluding bonuses or overtime)
  • Total annual pay (tell us what it is, e.g. W-2 box 1, gross before ยง125 or 401(k) reductions, etc)
  • Flexible spending account salary reductions (including medical, tuition and dependent care)
  • 401(k) deferrals: regular, catch-up (over age 50 at year end), total
  • 401(k) match
  • Profit sharing allocation (omit if Van Iwaarden Associates is doing the allocation)
  • Forfeitures allocated to the employee's account
  • Money purchase allocation (omit if Van Iwaarden Associates is doing the allocation)

Please provide the employee data in one file, with one line per person. We can read most IBM computer formats; our favorites (in order of preference) are 1-2-3, Excel, dBase and ASCII text.
